Medical History Form

We urge patients to fill out the Medical History form that is sent as a link on your confirmations. If you have any difficulty with this then we can print a form for you once you arrive for your appointment. It is important that this is completed every 6 months so the dentist, hygienist or visiting practitioner is aware of your medical history and they can treat you safely.

Please let us know in advance of any major changes, such as heart attacks, strokes, surgery or cancer treatment as this may affect treatment we can carry out for you.

Staff News

We look forward to welcoming back our receptionist Chelsie Steadman. Chelsie went on maternity leave in August 2022 after giving birth to baby Zachary. We are all looking forward to her return to the practice at the beginning of October.

Welcome to the team Maisie & Teya!

You may notice some new faces around the practice when you come in for your next appointment. Maisie Spooner and Teya Harper have joined the team as Trainee Dental Nurses and are currently working towards their exam. They will assist the dentist’s qualified dental nurse and work alongside the hygienists whilst training and will sit their exams next year.

On the 3rd October we held a very successful MacMillian coffee morning to raise money for cancer support. Our staff made some wonderful treats which were offered to our patients who kindly donated to the charity.

Every penny we raised will help support people living with cancer.

Have you ever thought about having straighter teeth?

On Thursday 16th November Dr Kiran Shankla will be running an exclusive open day on the benefits of having your teeth straightened. Teeth straightening can now be carried out in 6-12 months using a clear, removable aligner which progressively changes the position of your teeth. The aligners are invisible and discreet which allows you to continue with your daily regime without any lifestyle changes.

As well as straightening your teeth, there are many health benefits such as:

  1. Making it easier to clean your teeth
  2. Making it easier to chew
  3. Preventing jaw strain and pain
  4. Making you less likely to suffer from dental decay and gum disease.

On the open day we will take a 3D digital scan of your teeth so that you can see a before and after image of how you would look with your teeth straightened.
